Absence Request Form

Absences for the following reasons can be excused after the absence:

- SEVERE ILLNESS OR MEDICAL EMERGENCY

- DEATH IN THE FAMILY

- CHILDBIRTH IN THE FAMILY

Absences for the following reasons are normally considered excused with 24 hours notice:

- TUTORIAL/RETEST APPOINTMENTS WITH TEACHERS

Absences for the following reasons are normally considered excused with 14 days notice:

- MEDICAL SPECIALIST APPOINTMENTS (REQUIRES DOCTOR’S EXCUSE NOTE)

- PRIVATE LESSONS

- WEDDINGS

- FUNERALS

- RELIGIOUS HOLIDAY

Any other events must be submitted with 31 days notice in order to be evaluated by a director to determine whether the absence is excused or unexcused. An absence for any events submitted without 31 days notice will be considered unexcused.

If you are well enough to be in school, you are well enough to attend band rehearsals/performances.

Unexcused absences include, but are not limited to non-urgent/"check-up" doctor's appointments, organization/club meetings, work, family vacations, driver’s education, etc.

Work schedules, family trips, and other activities must not interfere with the rehearsal and performance schedule that has been published on our website. Students who enroll in other school activities such as athletics, choir, drama, etc. need to plan ahead and communicate with directors in order to avoid conflicts that cannot be resolved. The band directors will work together with the other teachers, coaches, and directors of various activities to give each student every opportunity to be a part of other groups.

Non-school sponsored activities, teams, organizations, or jobs should not conflict with the practice/performance schedule of the band.
